We welcome constructive feedback that recognizes positive conduct, sportsmanship, leadership, professionalism, and respectful communication. This form may also be used to raise concerns about behaviour, communication, or the overall game environment.

Please keep feedback fair, respectful, and based only on what you directly observed. This form is not intended for strike-zone complaints, pitch-by-pitch disagreements, or routine judgment-call disputes. Urgent safety matters or serious misconduct concerns should also be reported directly to League or District leadership.
